what adds an auto heading to my new files??

yeah when I open a new file like say a C/CPP file i get a automatic
header like so:

* Filename: std_index_sequence.c
*
* Description:
*
* Version: 1.0
* Created: 04/09/14 23:29:19
* Revision: none
* Compiler: gcc
*
* Author: YOUR NAME (),

I'm not sure if this is caused by a plugin I've installed or does vim
just do this??

the reason I ask is I want to set the name etc automatically if I can
