Use insert mode mappings, i.e. for the example Tim gave:
:inoremap <s-left> <c-o>v<left>
:inoremap <s-right> <c-o>v<right>

(For me in Windows gvim, this makes shift left in insert mode start
a visual selection. Further presses of shift left move my cursor by
words, while a simple left moves by one character at a time. The same
is true for right).
